Primary Responsibilities
Clinical Duties :
- Review referrals, read records, gather information, and complete client interviews
- Complete comprehensive strengths-based, client-centered, and trauma-informed clinical mental health assessments (IM-CANS) to adults at skilled nursing facility and specialized mental health nursing facility (SMHRF) settings
- Participate in development of Transition Service Plans (CSPs)
- Complete IM-CANS/IATP (Integrated Assessment and Treatment Plan) review acting as LPHA for staff in Behavioral Health division; providing feedback and coordinating with assessor as needed
- Cross-train and provide backup coverage to Front Door assessment team by monitoring referrals and completing assessment within 24 hours of referral to meet grant requirements
- Routinely consult with other Behavioral Health team members including employment services, psychosocial rehabilitation, SUPR/ MISA, drop-in staff, and psychiatry to address specific client related clinical issues and treatment recommendations
- Maintain a high level of documentation quality and compliance , including progress notes and integrated mental health assessments and treatment plans (IM-CANS).
- Ensure follow-up with assessor so that IM-CANS are signed and completed within 14 days
- Participate in clinical and administrative supervision as directed, to receive and offer feedback, for performance evaluation, and to seek clinical consultation as needed
- Interpret and communicate, at the service delivery level, Center policies and procedures
- Meet or exceed productivity target estimated at 50 -60 %
- Participate in orientations, all staff trainings, and training and development seminars
- Submit, in a timely and accurate fashion, schedules timesheets, mileage and other reports required by the Centerâs policies and procedures
- Preferred Masters Degree from an accredited College or University in Social Work, Psychology, Counseling or closely related discipline
- Strongly preferred LPHA in Illinois (LCPC/LCSW), licensed to practice independently
- At least 1 year experience completing comprehensive mental health assessments for adults with serious mental illness
- Demonstrated e xperience working with individuals transitioning out of facilities, and/or individuals experiencing chronic homelessness, complex medical issues, and/or substance use difficulties
- Pay Range: $60,000 - $75,000 per year
