Primary Function/General Purpose Of Position
The Conduit Flex RN Specialist will be responsible for providing patient services in both Transfer Center and Nurse Triage business units, while maintaining the core values of flexibility, innovation, agility, continuous improvement, and promoting change. Sustain a working understanding of regulatory requirements and accreditation standards within each department. Support leadership in compliance efforts to meet and sustain the regulatory compliance efforts of the department on behalf of the client. Maintain performance standards that prioritizes safety, quality and experience and coincides with the organization's mission and identified key strategic or performance initiates. Be mindful of departmental policies and procedures. Ensure all individual training and education requirements are fulfilled in a timely basis. Work with department managers and supervisors to identify and share safety best practices.
Essential Job Functions
- Demonstrates knowledge of regulatory components to include but not limited to EMTALA, Ethics and Compliance, quality initiatives, and HIPAA. Reports risks related to safety, compliance as well as operational inefficiencies using defined Chain of Command in a timely manner and offers recommendations for resolution or improvement if applicable.
- Maintains active RN licensure within the states services are provided.
- Ensures accurate, timely documentation in the EMR (Electronic Medical Record) according to best practice, guidelines, or workflows.
- Responsible for other tasks and analysis as requested /directed.
- Coordinates functions in order to identify appropriate level of care, accepting provider, consultation and access from all potential referral sources to include but not limited to hospitals, clinics, extended care facilitates, and physician offices to initiate acceptance and expedite decision making relative to a potential access; Focusing on quality, safety and efficiency.
- Works directly with clinicians at each client facility to ensure communication of accurate clinical presentation, timely access, appropriate bed assignment, and accurate patient information documentation to assist with an appropriate final destination plan of care at receiving facility using critical thinking, clinical decision making as well as established workflows.
- Works directly with providers and other healthcare providers at each client facility as well as client footprint facility to ensure timely acceptance and access to appropriate level of care/bed assignment using applicable workflows
- Provides telephonic triage to assist callers to determine the most appropriate level of care needed for the current situation expressed or assessed, following workflows and utilizing protocols/resources to provide supportive service to patients and customers.
- Provides telephonic triage or requested support and / or virtual monitoring. Offers subsequent recommendations, education or care advice using decision making tools, clinical judgement, and defined workflows.
- Participates in care coordination, by partnering with customers to reduce readmissions, enhance chronic disease management, manage health risk and injury reporting. Schedules provider appointments and facilitates provider communication.
Licensing/Certification
Active Registered Nurse (RN) – Required - license must be active in state of residence
Education
Associate Degree of Nursing or diploma (ADN) - Required
Work Experience
Bachelors degree of nursing (BSN) – Preferred
1 Year Of Acute Care Experience – Required
3+ years of acute care experience – preferred
Healthcare contact center – preferred
Training
Epic electronic health record – preferred
Genesys contact center – preferred
